[BUG] Model name not found when using control net

Describe the bug:
Model name displays as 'Not Found', but shows correct hash. I suspect this is due to using a control net tile model for upscaling and has to do with the way the program handles metadata containing multiple models.

1.9.1

Tool:
Forge WebUI, which I realize is not explicitly supported, but it is based on A1111

To Reproduce
Upscale image in Img2Img using SD-Ultimate-Upscaler and Control Net Tile
(I did confirm that the issue is specific to using Control Net and does not occur when using just Img2Img upscaling or SD-Ultimate)

Expected behavior
Base model name shows in image previewer next to model hash.

It would be nice if additional models like upscaler and control net could be displayed as well, but that would be more of a feature request.

Additional context
I also noticed that a new model line for the Control Net tile model gets added for each base model that I use it with. That is a probably separate issue though.
